PATIENT REGISTRATION – COMPLETE THESE FORMS PRIOR TO YOUR APPOINTMENT
If you have already scheduled an appointment but have not yet filled out your Patient Registration Forms, please log into your Patient Portal below to complete your forms online prior to your appointment.
If you are unable to complete your forms via the Patient Portal, please arrive fifteen minutes before your appointment to accommodate completing the paperwork which will be provided by one of our Patient Care Coordinators.
Please note that if you are an existing patient, you may be asked to complete a new set of Patient Registration Forms depending on the last time you had an appointment with us.
For security and verification purposes, our Patient Care Coordinators will also request a form of identification and your insurance card when you check in for your appointment.

Insurance
If your insurance covers dermatological treatments, you will receive the benefit of reduced out-of-pocket costs. Because insurance policies vary, we review each case individually to determine the best course of action for you. Additionally, some plans require a referral from your primary care physician so it’s a good idea to check with your insurance company for specific requirements.
